PROVIDING A PERSON CENTERED APPROACH TO INDIVIDUALS WITH THESE CONDITIONS THROUGH ASSESSMENT OF NEED, A PROFILE OF THE CLIENT, SOCIAL INTERPRETATION, SUPPORT GROUPS, ADVOCACY, BEFRIENDING, SOCIAL AND LIFE SKILLS COACHING AND WORK MENTORING.
